Classification of HIV infection

WHO recommends distinguishing four stages of the disease:

• initial (acute);
• persistent generalized lymphadenopathy;
• AIDS-associated complex as pre-AIDS;
• full-blown AIDS.

In addition, recently it has been proposed to identify a 5th stage of the disease - AIDS dementia.

In Ukraine, the clinical classification of HIV infection by V.I. Pokrovsky (1989) has been adopted:

• I. Incubation stage;
• II. Stage of primary manifestations;
  • A. Acute febrile phase,
  • B. Asymptomatic phase,
  • B. Persistent generalized lymphadenopathy.
• III. Stage of secondary diseases:
  • A. Weight loss of less than 10%, superficial fungal, viral, bacterial lesions of the skin and mucous membranes; herpes zoster; recurrent pharyngitis, sinusitis.
  • B. Progressive weight loss of more than 10%; unexplained diarrhea or fever for more than 1 month; hairy leukoplakia; pulmonary tuberculosis; recurrent or persistent bacterial, fungal, viral, protozoal lesions of internal organs (without dissemination) or deep lesions of the skin and mucous membranes; recurrent or disseminated herpes zoster; localized Kaposi's sarcoma.
  • B. Generalized bacterial, viral, fungal, protozoal and parasitic diseases; Pneumocystis pneumonia; esophageal candidiasis; atypical mycobacteriosis; extrapulmonary tuberculosis; cachexia; disseminated Kaposi's sarcoma; central nervous system lesions of various etiologies.
• IV. Terminal stage.

In young children, the following stages of the disease are consistently identified:

• subclinical;
• lymphadenopathic;
• localized opportunistic infections;
• generalized opportunistic infections.

At the subclinical stage, clinical manifestations of HIV infection are completely absent.
